Medical Directorship

The role of the medical director in the post-acute and long-term care setting is a critical one. It requires specialized physician knowledge, training, and experience – as well as medical management skills that can effectively balance patient care and business success.

Our medical director services are available to our partnering organizations across the care continuum, including skilled nursing, inpatient rehab and assisted living care settings. Our Medical Directors focus on working with the post-acute operations and clinical teams to streamline processes utilizing clinical best practices, attend QAPI Meetings to assist team with improving  performance of key indicators. Medical Directors provide ongoing education and training with staff to ensure the patient satisfaction and quality outcomes.  

Primary Care

Daiya Healthcare introduces a unique model of care, offering streamlined processes and evidenced based medical treatment. Our Internal Medicine Providers are onsite daily at our partnering facilities, collaborating with staff to deliver the highest quality of care possible.  The primary care physician supports new admissions, history and physicals, advanced care planning, regulatory visits, care coordination and care transitions/discharges from facility. With direct provider oversight we are able to treat a wide variety of acute and chronic conditions in-house, reducing hospitalizations. Our dedicated team of post-acute care providers provide 24/7 call coverage to address the ongoing patient care needs.

Specialty Care

The Specialty Care Team at Daiya Healthcare includes Board-Certified Physicians, Nurse Practitioners, Physician Assistants, Pharmacists, Registered Nurses, and Medical Assistants. Along with being trained and experienced in their respective fiends, all Daiya Healthcare specialists undergo a proprietary residency training program and on-site job orientation individualized for each care setting. Our team is trained in the following specialties:

Our Daiya Pharmacists work with our partner facilities to

  • Collaborate with facility in-house pharmacy consultants 

  • Review medications and reduce redundancies 

  • Reduce overall pharmacy costs 

Quality Initiatives

Daiya Healthcare, through quality initiatives, works to ensure coordination of care. Our medical standards are delivered consistently and efficiently in compliance with the Centers for Medicare and Medicaid Services, The Centers for Disease Control, and other regulatory agencies. 


Our same day documentation commitment provides an up-to-date and comprehensive care plan, decreasing delays in treatment and improving coordination of care. Clinical outcomes are improved by establishing streamlined processes as the patient transitions between care settings. 

Daiya Healthcare strives for a less than 10% re-hospitalization rate during the first 30 days post hospital discharge. By partnering with Daiya Healthcare, we can not only improves outcomes but can also generate cost savings to our partner facilities through effective processes and utilization of resources. 

Telemedicine

At Daiya Healthcare, our goal is to provide accessible healthcare services 24 hours per day. Telemedicine is an integral part of the post-acute care services that we provide. Telemedicine offers a variety of benefits to the patients in our partnering facilities, including:

  • Onsite Medical Assistants facilitate telemedicine visits with providers, enabling facility staff to focus on patient care 

  • Quickly addresses health issues, leading to improved patient outcomes and reduced hospital readmission rates

  • Telemedicine services provide additional revenue to our partnering skilled nursing facilities.
